  1) How many mortgage payments have you made in the last 12 months that were 30+ days late?

  2) How many mortgage payments have you made in the last 12 months that were 60+ days late?
  3) How many mortgage payments have you made in the last 12 months that were 90+ days late?

  4) Have you filed bankruptcy or foreclosure in the last 7 years.

  If so, have you paid according to the workout plan and your mortgage for the last year?

  5) Have you had any Bad Accounts( Collections, Judgements, Tax Liens, Charge Offs, or Delinquent Property ) within the last two years?

  6) If you have had any Bad Accounts in the last 5 years without six months of satisfactory payment:
A. How Many of these Bad Accounts are still open?
B. What is the outstanding balance on these Bad Accounts?
  7) What is your monthly gross income?

  9) How many companies have you had credit with for at least 1 year?
( including Major Credit Cards, banks, department stores, mortgage and finance companies )

  11) How many installment and revolving payments have you made in the last 12 months that were 60+ days late?
  12) How many installment and revolving payments have you made in the last 12 months that were 90+ days late?
